Improving communication processes when a person moves from home-based hospice care to the residential care setting.

Last year the Residential Care Team at the Otago Community Hospice (OCH) began developing a transfer of care process to help ensure a smooth transition for people moving from their homes into residential care facilities.

The goal of this Quality Project was to create a more structured and effective handover of information to improve continuity of care both during and after the transition to residential care.

During this Quality Project it has become clear that the more information that is shared with the residential care facility, the better transition for the patient and their family.  

To ensure that the transfer of care process continues to evolve and improve, it is important that we continue to evaluate its effectiveness both for the facilities and the patients themselves. By doing this, it ultimately ensures the best possible care for patients during their transition to residential care. 

Feedback sought.

We welcome any feedback from residential care staff who have been part of a transfer for patients that have been on the OCH service. This includes patients who may have moved from their home in the community to residential care and those that transfer from the OCH Inpatient Unit to residential care.

When you are involved in admitting patients into residential care from either setting, is the information being provided by the OCH teams useful, are the transfer documents helpful or is there more information needed?

We are keen to know how it is going.

We are hoping to include the different perspectives of staff involved to ensure we identify any gaps in the information that is shared. So if you see us about in your facilities, feel free to grab us for a chat. You can also share your thoughts and suggestions on the transfer form/process for patients who are moving from their homes into your facilities.
