More People, More Equity, More Sustainability
Over the past 12 months, the Urban Team has successfully transitioned to a shared care coordination model, with two teams of four care coordinators working collaboratively to provide and coordinate care for patients within their designated areas.
This represented a significant shift from the previous case coordination model and was introduced to improve equity in care delivery and address workload disparities identified following the implementation of domiciliary care.
Building on the success of this approach, we are now reviewing our rural care coordination models to explore how similar principles can be applied to ensure equitable access to services for all patients enrolled in our programme.
As we continue to refine and strengthen our service delivery, we greatly appreciate the team's flexibility, willingness to embrace change, and solutions-focused approach. Their ongoing commitment to continuous improvement has been instrumental in helping us deliver more equitable and sustainable care.
