Urban Community Team Shaping Their Future Together

Within the urban community team, we are working through the recommendations of the six-month review of extended services, as it becomes 'business as usual' and as part of this process we are looking to move to a shared model of care coordination.

"The community team are the team facing the daily challenge of managing their very variable workloads. They are best placed to navigate the best way forward," says Ginny. "As Becs would say, 'hive thinking' will provide the best solutions."

The team are doing this with the guidance of an expert external facilitator -Jan Samuel. Jan has worked in human resource management for over 30 years and is an expert in mediation, change management, and strategy development, policy and procedures.

Jan will be leading workshops throughout November with the whole community care team to help us shape how this new shared model will look and work in our daily practice.
